General & Family Physician 's  Response
Hi,

I have gone through your medical history carefully and understood your concern.

From what you are describing, this might be a case of possible anal abscess/fissure or fistula.
As you are planning to travel back to United States by plane, I recommend to go to ER there in Philippines and get it checked there. Ask to prescribe some temporary medications to relieve your pain until you go back to US.

For the moment, I can advise to keep good local hygiene and take OTC anti-inflammatory meds/painkillers like ibuprofen or mephenamic acid.

Wish fast recovery!
