Suggest Treatment For Constant Nasal Drainage

first of all , I am a dual transplant recipient (kidney and 3 mo. later pancreas) 12 yrs. ago. I am having problems with my throat. voice is changed to raspy and lower. I am a singer and this proves detrimental. It feels like I have been yelling too long at a sporting event. But there has been no yelling. I saw ENT Dr. at VA and she said I had Acid re-flux while sleeping. Put me on omeprazole 20mg. and Flonaze nose spray. It hasn t helped that I can tell. My breathing is a little restricted and my singing is shaky at best. I tried to read up and find out something/anything but only came up with Hyperthyroidism. But I really have no clue. She stuck a camera into my nose and we looked together at my vocal chords. they appeared to be fine but the two spots just in front of the vocal chords were(above)inflamed, and (below) swollen or endemic.Constant nasal drainage and constantly clearing my throat . When I clear it, I can sing just fine until it gets full of nasal drainage again. getting real frustrated by now.

Otolaryngologist / ENT Specialist 's  Response
Hi,

Thank you for your query.

1. If you are on immune suppressants, you are likely to get chronic laryngitis and chronic sinusitis.

2. The sinusitis may be the source of the post nasal drip. Get a plain Sinus CT done.

3. The inflamed spots in front of the vocal cords may be due to reflux or early vocal nodules. Increase the dose of omeprazole or have one on an empty stomach half an hour before breakfast. You may also try other proton pump inhibitors such as rabeprazole, pantoprazole and so on (sucralfate, mucaine gel). A video laryngoscopy with a Stroboscopy may help delineate any (Reinke's) edema of the vocal cords.

4. Get a Gastroenterologist's opinion. Get an abdominal Ultrasound (USG) done to rule out any hiatal (sliding) hernia. Rule out H.pylori infection.

5. If you have a PND with sinusitis, Balloon Sinuplasty is a good option.

6. Are your thyroid function tests normal? You may follow up with the reports at bit.ly/Dr-Sumit-Bhatti

Regards.
