Suggest Treatment And Diet Chart For High Blood Sugar Levels

I am having High suger and I dont know what type of diabeties I have I have got diabities since 4 years and my kidney is normal and I will be getting tayered and foots feeling like burning and I take 2 times wheat chapathis and afternoon rice and I am taking Insulin 2 times day morning 40ml and night 30ml plz give solution to me
