Suggest Tests To Diagnose Partial Impotence

man, 19, smoker.i seem to have developped partial impotence, as it is hard to develop or maintain an erection. as soon as stimulus is provided (masturbation or intercourse) i can go through, but at any interruption i go limp and my penis refuses to get hard again. it is also very hard to develop an erection even if sexual desire is present. i have been cheated on so the answer may be psychological but i need to consider all options. it is causing strain in my present relationship and i am asking for help.
