Internal Medicine Specialist, Dr. Anila's Response
in this patient, there are 2 things to consider. one is sepsis which causing low blood pressure. Patients with terminal cancer are prone for sepsis. I would suggest a complete blood count and if WBC is high or low, search for source of infection and start antibiotics. another possibility is adrenal failure due to metastatic deposits in the adrenal gland. in which case I would suggest 8 am cortisol with or without cosyntropin test and if suggestive, start steroids.
