General & Family Physician 's  Response
hello
welcome to HCM,
your liver is enlarged probably due to pancreatitis.oedematous pancreas causes mild outflow obstruction.
but i need your ALP value to prove this.
for now,
Stop alcohol strictly
Dont smoke.
take some pancreatic enzyme complex,PPI,LIVER TONIC,TAB UDILIV daily.

NOW for your fluctuating level of high blood sugar at present you should start with insulin later when it is normalised switch over to oral hypoglycaemics.
consult an endocrinologist for proper dosage of basal and pp insulin.
and also to check the other systems and retinoscopy.
hope this is helpful.
regards.
