Suffering With Severe Acne. Is There Any Need Of Topical Creams While On Oral Isotretinoin Therapy?

i have few queries regarding use of oral isotretinoin in severe acne treatment .please guide me in detail. 1.Is there any need of topicals while patient on oral isotretinoin therapy .being it covers all the four possible factors that predispose to acne. 2.Your comments on additional use of oral antibiotic like macrolides initially with oral isotretinoin. 3.What is the ideal starting dosage of oral isotretinoin and duration of therapy.

Thanks for asking on HCH forum,
Usually if patient is on ISOTRETINOIN no other oral antibiotic is taken .you can apply local retin A cream at night and clindamycin lotion at day.
   ISOTRETINOIN  used mostly for cystic acne .it is related to vitamin A , and is found in small quantities naturally in the body. Isotretinoin s best-known and most dangerous side effect is birth defectsThe dose of isotretinoin patients receive is dependent on their weight and the severity of the condition. High-dose treatments are administered between 0.5 mg/kg/day to 2 mg/kg/day (usually at 0.5 to 1 mg/kg/day,divided into two doses), for a total treatment of four to six months
