General & Family Physician 's  Response
Dear patient,
Thanks for contacting HCM
I have read through your problem and i think you are suffering from Irritable bowel Syndrome (IBS). IBS commonly causes cramping, abdominal pain, bloating, gas, diarrhea (frequent loose stools, often with an urgent need to move the bowels) and constipation (difficult or infrequent bowel movements). Some people with IBS have constipation , others have diarrhea , and some people experience both. Symptoms you have mentioned correlate with those which occur in patients of IBS.
You have not mentioned your age but it generally occurs between age of 15 to 35 years and stress has been said to be one of the causes of IBS. The good news is that IBS does not cause permanent harm to the intestines and does not lead to intestinal bleeding or cancer.
If you were my patient and had walked into my clinic i would have ordered some investigation like CBC (complete blood count) to rule out Anemia and infection, stool routine, stool for occult blood, ESR, serum calcium and serum Vitamin D3. I would advice to increase fibre intake and suggest certain psychological therapies like Relaxation training including yoga and meditation, Cognitive behavioral therapy (it involves identifying the triggers for symptom exacerbation, understanding the patient’s response to symptoms, and teaching more adaptive ways of responding).
In Pharmacological treatment i generally prescribe-
Tab Nortryptyline 25 mg once a day
Tab Loperamide 2 mg once a day (when you have diarrhea)
Tab Rifoximin 400 mg twice a day for 14 days then once a day for 14 days.
Any probiotic like Bifilac HP capsule once a day
But Non pharmacological to reduce stress is more important and you should start with them as soon as possible.
I hoped this has helped you
Regards
