Pain Outside Anus Towards Scrotum Like An Abrasion. Was Told It Is A Fissure, Offered Cream

I have had a pain in the area just on the outside of my anus towards the scrotum. Almost like an abrasion. A dr once told me I had a fissure but only offered a cream to soothe the pain. It has been over 10 years of this problem off and on. It gets very sore when I ride a bike or have diarrhea. It is also always wet around my anus until I wipe, then I notice a yellowish substance on the TP.

I would come up with these possibilities for this pain around anus, these includes.
1 Fissure in anus , diagnosis can be confirmed by physical examination by a surgeon.As you mentioned that , when you wipe there appear a yellowish substance over paper so this may be a oozing from anal canal . This is a characteristic feature of fissure in anus.
2 Hemorrhoids , diagnosis can be confirmed by physical examination.
3Dermatitis , eczematous type , diagnosis can be confirmed by physical examination by a dermatologist.
In my opinion consult a surgeon and get his opinion.
