Nephrotic, Cold And Cough. Steroid Dependent And On Omnacortil. What Does Protein Level Show?

my son is 6 and half year old and hi is steroid dependent nephrotic child at present he is on 10 mg omnacortil and 100 mg decaris both alternate day with 2.5 mg invace but his protien is 3+ from last 7 days he is also suffering from cold and cough what is future of my child.and what is option for getting remission is it proper diagnosis

Hello,

Your son has been diagnosed as steroid dependent nephrotic syndrome. At present he has proteinuria of 3 + and is also suffering from upper respiratory tract infection. Any form of infection in these patients can cause relapse. At present your child is in relapse and for treating that the dose of steroids ( omnacortil) will have to be increased till he again goes into remission. This has to be done after consultation with your paediatrician / nephrologist.

The prognosis in these patients is usually good and besides steroids, levamisole (decaris) is a very good option. But I can answer your question regarding diagnosis and prognosis better if you can provide me answers to these questions -

1) When was he first diagnosed with nephrotic syndrome?
2) How frequently is he having relapses?
3) Can you provide me datails of other investigations if done- C3 levels, Kidney biopsy, any other special tests?
4) After adding levamisole (decaris) , could your doctor decrease the dose of steroids to maintain remission?
5) How long has he been in remission with this dose of medicines? Also can you tell me his weight - that will help me in knowing his exact steroid requirement?

In the mean time I advice you to visit your paediatrician/ nephrologist for the treatment of the current episode of relapse. Also I will recommend you to go to a paediatric nephrologist (if there is one in your area) for a detailed evaluation as this is not an ordinary case of nephrotic syndrome.
