MRI Shows Large Right Posterior Paracentral Disc Herniation And Spondylolysis. Do I Need Surgery?

I have a mri finding of a large right posterior paracentral disc herniation at L3-4 which migrates caudally into the right lateral recess at L4 and displaces the right L4 nerve root. I also have spondylolysis at L5 and grade ll spo atndylolisthesis of L5 on S1. Bilateral neural foraminal stenosis at L5-S1 wich is moderate to severe on the right and moderate on the left and there is contact on the exiting L5 nerve roots bilaterally. Do i need surgery or can this be fixed with therapy.

as your problem lying large rt paracentral disc herniation at L3/4 won"t requires open surgery,but grade 2 nd spondylolisthesis definetely require fixationALIF/PLIF.depend on.because it can cause further canal stenosis.may lead to cauda equina syndrome,better to go open surgery
