It seems that you are low in carbohydrate
and protein .. I suggest you to follow diet plan
as follows to gain weight and nutrition..
Morning - milk + banana + egg + Bread / carbohydrate food
11am - fresh juice ( alternate fruits )
1pm - Lunch rich in green leafy vegetables
4pm - Protein rich foods like ( gram.. nuts etc)
8pm - banana + Bread
Also add multivitamin
in morning , calcium tablet in afternoon and iron with vitamin c
at night to get adequate nutrients..
follow this for one month and you will have definite improvement..
