Is It Safe To Take TB Medication In Empty Stomach?

Hello doctor..my father inlaw is suffering fm stomach TB and he is prescribed to take akt4..he not able to eat..there is loss of appetite.he is afraid that it harmful to take med empty stomach..please gv ur advise if any alternative is thr also please let us knw how soon he will be fine n normal n will start taking his normal diet..

AKT 4 is a combination of rifampicin450+INH 300 mg+pyrazinamide 750 mg ( 1 tablet)+ethambutol 800 mg (1 tab).
Usually anti tubercular drugs ( 1st line ) is advised on empty stomach so that food don't interfere in their absorption . Sometimes due to many reasons few patient can't take these drugs on empty stomach.
In such type of cases I advise my patient to take 3 hours after meal .
In spite of this many patient complaining that they are feeling nausea and belching , so I advise them to take pantoprazole on empty stomach twice in day i.e. one before meal and akt4 after meal .
Still few patient feel nausea so for first few days they need intravenous infusion of normal saline ( if not contraindicated ) with I/V ranitidine or sodium bicarbonate .
For stomach tuberculosis usual course of ATT is 6 - 9 months depending on the condition of patient , severity of symptoms , blood reports and treating doctors.
Intensive treatment is for first 2 months and your father in law will start normal food within first 5-7 days after ATT ( Anti Tubercular Treatment).
Normal diet , you can start once patient want it.
