Is ESR Level 46 High For 71 Year Old Woman?

My ESR is 46 I have had Polymylagia for over 10 years. I am female and 71 years of age. This week a doctor at my GP Practise increased my prednisolone to 15mg and said to increase to 20mg in 2 weeks if neck pain/shoulder pain hasn't eased. He doesn't seem to be taking the fact I also have neck arthritis and also neck dystonia (in remission but muscles still pull and I have a head tremor) into account. Please can you advise as I am very worried about being on such a high dose and since increasing my dose to 15 from 8mg that has made my head tremors worse since Wednesday and really don't want to go up to 20mg as he is suggesting!! I am a bit reluctant to contact him again. On the internet I see an ESR of 46 is not too high for a woman of 71???

Hello,

ESR and CRP (C-reactive protein) in Polymyalgia are often abnormally high. ESR which is commonly called a "sed rate" is a test that measures how much inflammation is in the body. Normal result for women over 50 years old is less than 30.

Polymyalgia rheumatica is usually treated with a low dose of an oral corticosteroid (prednisone). At the beginning of the treatment the dose of the corticosteroid is usually 10 to 20 milligrams a day. Your doctor can slowly decrease the dose to find the lowest dose that keeps your symptoms and the results of sed rate (ESR) and CRP under control. If patient symptoms do not respond initially to this dose, the prednisone dose should be increased to 30 milligrams a day.

Please note that corticosteroids can cause side effects like:
- osteoporosis which is the loss of bone density and increases the risk of bone fractures,
- high blood pressure and cholesterol level,
- weight gain which is very common,
- thinning of the skin,
- cataracts
You should be checking for these problems by your doctor during follow-up visits. To help prevent osteoporosis Your doctor should prescribe You calcium and vitamin D supplements.
