I'm A 62 Year Old Female. I've Had A Kidney

I'm a 62 year old female. I've had a kidney transplant for 15 years. I was in hospital 2 weeks ago for a night on saline IV because my sodium level was down to 122 and my creatinine up to 166, (last week it was down to (122). They took me off of ramipril, 1/2 / of Diltiazem and off of furosemide to increase my sodium and lower my creatinine, which it did. I have been shaking really bad. My hands have been shaking so much and and I feel like I'm quivering inside and I'm short of breath if I go up a flight of stairs. My BP is pretty normal. Any ideas on what it could be.
