I Have Severe Shoulder Pain Where The Shoulder Meets The

I have severe shoulder pain where the shoulder meets the bicep. It also is in the back by my shoulder blade. If I rest my arm on my legs while sitting it makes it worse as well as when I do any movement with my elbow bent, like its resting on my knee. I've tried ice, heat, not moving it for a hour and the pain does not go away. I wear a 25mg fentanyl patch as well as take 7.5-325 hydrocodone 4 x daily, for a bulging disc in my back. None of this has touched the pain. I've tried aleve as well as flex aril that was prescribed 8 months ago for my back issue. I just moved to a new town and don't have a family dr. I'm afraid if I go to the emergency room I will just get x rays and be told to follow up with a family dr. I've had this pain for almost 3 weeks. Is there anything else I can do or will a emergency room do a CAT or MRI scan if x rays are negative?
