I Had Severe Back Pain A Couple Of Years Ago.

I had severe back pain a couple of years ago. I went to see a nurse practitioner (because I can never get in to my doctor) and she sent me to a pain specialist. I did exersizes but I started having sciatic pain and it was too painful to do them. A friend suggested I go to a chiropractor because it helped her husband. After going 3 times a week for 4 weeks the chiropractor re-evaluated me. My sciatic pain went away but he said according to the tests that he did, my back was worse. I stopped going because of financial my situation.
I've been having a lot of back pain recently so I asked my new doctor to order an mri. When he received the report he had his assistant call and tell me that he wants me to see a pain management specialist. I don't know if I should try it again or if I need to have surgery. I'm just really tired of not being able to simple things like bend over or step off the bus without my back going out.
I have a copy of the report. I understand some of it and some I don't. Can you help me understand it and give me some advice on what I should do?
