I Am Having A Problem With Being Constipated All The

I am having a problem with being constipated all the time. I have pain in my lower back and deep in my abdomen area making it hard to get up in the mornings and hard to walk and sit down on the toilet in the morning. It hurts to move around and when it eases up a little, I can get showered and dressed. It still hurts to try and bend over to tie my shoes.
I have brought this up to my primary doctor and he continues to tell me to lose weight and exercise and take miralax. My cardiologists says use fiber tablets. I have arthritis and have back problems due to an accident back in 1994. I am 5 ft 3 inches and weigh 235.
I feel there is something more wrong with me and i'm sure it has to do within my intestines and not just my back. Every bowel movement I force out is hard little balls and sometimes it fels like something is keeping them from coming out. I have been using prune juice and Dulcolax. And my stomach feels miserable a loy of the time. Usually in 5 or 6 days I will pass some little hard balls. I also take Tramadol to help with my pain.
Am I wrong to ask for some tests or for them to look into my problems? I know my body and for over a year I have been hurting and constipated and it is getting worse, not better.
