How To Treat The Tingling Sensation In The Cheeks And Teeth Grinding Habit With Tmj?

I am 23 and having severe Jaw issues, it constantly feels loose and as if it is being pushed forward. I get this tingling feeling in my chin a lot and it feels as though it is being pulled inward. My face is becoming less defined as my jaw seems to be moving forward and the resting place of my jaw is changing to that forward position close to my cheek bones. I have tmj and grind my teeth bad, but have been wearing a night guard for the last year to try and ease that. This has been happening drastically for the past 5 years and I don't know what to do, I have been to doctors and they don't do much for me but say try and stop grinding your teeth and take smaller bites of food, don't yawn so wide. I feel helpless that it is just going to keep getting worse until I can't do anything.
