Hi
Welcome to healthcaremagic
He has two problems
1) external rash over scrotum..most probably due to fungal infection due to moistness.Treatment is antifungal ointment
2)crying before passing urine is commonly due to urinary tract infection. It can be confirmed by urine test and then treated with oral antibiotic Give him lots of water which will flush bacteria and reduce pain quickly
For external infection don't wear him diaper for 2-3 days and keep open as much possible
Happy to help you
Best regards
