I have a patient with exophthalmos affecting specifically the left eye. She is relatively young, attractive and is very concerned about this cosmetic defect. Her Graves disease is currently well-controlled and she has no exophthalmus of the right eye. Decompression surgery seems to be appropriate in only the most radical of cases as the potential subsequent morbidity can be severe. Is there however eyelid surgery that might be performed that would perhaps in some manner minimize the disfigurement of this bulging by? As I consider this patient s appearance it looks as though the exophthalmus is made more dramatically apparent by a thickening of the upper lid and somewhat of a withdrawal of that lid simultaneously.
