General Surgeon 's  Response
Hello and thank you for asking HCM.

I have read your symptoms and i understand your concerns.

-history of 5 years of hard stools
-passing stools with anal pain
-presence of blood after the stools

These symptoms correlate with hemorrhoids (probably in your case).
Also an anal fissure may be there. Another posibility is anal or rectal cancer.(least possible)
Before jumping in conclusions,if i was your caring doctor i would first do a digital rectal examination to confirm the diagnosis of hemorrhoids.

I suggest you check up with a surgeon to eamine your anus, find the diagnosis and treat it.

Meanwhile here are some advice how to soften your stools. It will help with your symptoms:
-To minimize pain, soak in a warm tub of water for at least 10 minutes every day.
-Exercise regularly to prevent becoming constipated
-Have foods that are high in dietary fiber like whole wheat, brown rice, oatmeal, pears, carrots or buckwheat
-eat kiwis
-drink plenty of liquids
-avoid alcohol

If you have any other question feel free to contact me.
