Hello,
Thank you for your for your query.
I've read through your query and understand your concerns.

Pain after heart surgery lasts about 4weeks with full resolution by 6-8 weeks.
Bronchospasm (due to exercise/physical strain), after a bout of pneumonia, may last upto 10-14 days.
Chest pain on exertion and shortness of breath needs to investigated for new/current problems.

I would :
-examine you for any signs of heart failure, arrhythmia, bronchospasm.
- do an Electrocardiogram, echocardiogram, chest x-ray and a spirometry test.
-an exercise stress test to look for new ischemic events.
-do an angiography to check for any further progression of coronary disease.

Based on this, a treatment plan will be made.
If you continue to have chest pain & shortness of breath on exertion, please see your cardiologist right away.
Any giddiness, unexplained perspiration, persistent chest pain/shortness of breath even on rest warrants a visit to the ER.

Please get it checked. Events in August 2015 are highly unlikely to be causing symptoms now. New. Problems, as mentioned above must be ruled out.

Hope you feel better.
Wish you good health.
Please let me know if there is anything else I can help you with.
I find this answer helpful
