Varicocele is the varicose of the spermatic veins. It is linked to an absence or to a valvular
incontinence of the spermatic venous network, responsible for a retrograde flow towards the gonadal plexuses due to internal spermatic insufficiency. This dilatation, sometimes visible and palpable, is often associated with testicular hypotrophy. Varicocele is a common clinical entity, present in 15% of the male population. It is found in approximately 35% of patients with
primary infertility and in 81% of patients with
secondary infertility. The exact mechanism by which varicocele affects fertility remains indeterminate even if testicular lesions of variable intensity and worsening with time are evoked. The
surgical treatment of varicocele improves the quality of the sperm with a percentage of pregnancies obtained varying, depending on the series, from 20 to 60%, with an average value of 32.4%. A varicocele often produces no signs or symptoms. Rarely, it may cause pain.

Valsalva maneuver (The Valsalva maneuver is performed by moderately forceful attempted exhalation against a closed airway, usually done by closing one's mouth, pinching one's nose shut while pressing out as if blowing up a balloon. )
Varicocele is a common condition that can cause the infertility of the couple. The spermatic profile is that of an oligoasthenotératozoospermia (no spermatozoid in sperm). The surgical cure significantly improves the number and mobility of
spermatozoa in non-azoosperm patients.
I therefore believe that surgical treatment is indispensable not to fight pain but to avoid sterility.
