How Can Severe Pain In The Knees Be Treated?

I am diagnosed with Chondromalacia patella Age: 27 Nationality: Indian Diagnosis: 2 years ago Weight: 170 lbs/ 77 Kgs Gender: Female No other medical history I was physically very active (cardio in gym, high intensity Zumba) before my knee pain started, since my diagnosis I am physically very inactive 2015 Aug : Diagnosis of CMP. I had my physiotherapy for 1 month, but it did not help a bit, it only worsened, so I stopped it. I was in medication for 2 months after my diagnosis. I took plenty of rest and did no physical activity for the next whole year. The pain ameliorated but I still had pain while sitting standing, going upstairs and down stairs 2017 Jan: I tried physiotherapy again, only to realize that it worsened my condition again, so I stopped it. Now I have more pain in the knees thighs and glutes. I went to the doctor and he suggested PRP injections, which are very expensive. 1. I am wondering if anyone could suggest any other treatment ? 2. I also want to know about the recovery time, if I have to take a leave from my job for few weeks after the injection , 3. Do I have to get the injections periodically? 4. Should I go to physiotherapy again? 5. What is the success rate of PRP for chondromalacia 6. Does my CMP lead to osteoarthritis? 7. I think I also overpronate a lot when walking, is this adding I have been in depression because of this majorly since I was diagnosed. The pain is intense on some days. I cry everyday because of this. I haven t found any treatment that works for me. Working out was my favorite activity and I can t do it anymore, let alone working out I can t even walk with out pain these days. I hope some helps me. I d be indebted to them

General Surgeon 's  Response
Hello,

In your case, there are a few possible causes and if there was no injury, then some other chronic disorder may be present. Without more information it is not enough to determine the diagnosis. It could be a hematoma (bleeding in the tissues) or a swollen knee joint capsule. If it's in the back of the knee, then it could be a Baker's cyst. Clinical examination (and perhaps fluid aspiration if there's any) would help to determine the diagnosis. Either an orthopedics specialist or your primary care physician can do that.

Until then you can try to apply ice packs (not directly on the skin - wrap them with a towel) and perhaps use some painkillers starting with paracetamol (acetaminophen) and advancing to NSAID if paracetamol is not enough and there's no contraindication for NSAID.

Hope I have answered your query.
Let me know if I can assist you further.

Regards,
Dr. Ivan R. Rommstein,
General Surgeon
