How Can Severe Pain In The Ankle Be Treated?

I was in a near fatal car accident 3 1/2 years ago and my ankle needed to be completely rebuilt, large bone was fractured in 2 places, pins, screws and plates was placed. 2 weeks ago I started having terrible pain like the skin tearing and burning, now today every time I take a step it is a 10+ pain level pain. What could cause this pain to show up now?

I can understand your concern.
At present, your pain is mostly due to ligamental or soft tissue related.
But better to consult orthopedic doctor and get done clinical examination and MRI of ankle joint to rule out fracture or other serious problems.
You will need painkiller and anti inflammatory drugs.
Immobilization with splint is also beneficial. Leg elevation in the bed is also helpful in reducing pain.
