How Can Severe Earache Be Treated?

I have been on Z pack for 3 days (Zithromycn) for an earache. I took the last pill at ! pm, the earache is very bad at this point in time, can I take 500MG of Clarithromycyn at 1am. My physician said I could (earlier this eve when the earache was getting worse) but I am very worried about this.

Hello and Welcome to 'Ask A Doctor' service. I have reviewed your query and here is my advice.

From your explanation it looks like ear infection only and in that case taking a dose of antibiotic cannot immediately relieve pain.
So if you have pain you need to take a painkiller like Ibuprofen and in case if pain is very severe you can take Narcotic painkiller Tramadol.

It will help in relieving pain.

As far as taking antibiotics is concerned you should get your ear examined again and you can be advised antibiotics for at-least a course of 7 days and along with it painkillers and antiobiotic eardrops can be advised..

If the ears feels clogged use decongestant nasal sprays like Oxymetazoline or Xylometazoline.

If there is fluid or pus in ear it has to be manually drained.
