How Can Myelofibrosis Be Treated?

I have advanced stage Myelofibrosis. I see my Oncologist weekly for blood samples and receive procrit shots and periodic blood tranfusions. I have also been on Jakafi for about a year. Nothing seems to work anymore. I am fatigued all the time. I would like to know the signs of end stage just prior to death.

myelofibrosis is a disease in which hyperplasia of megakaryocytes which produce specific factor leading to fibrosis.
marrow support and stem cell transplant may be curative but its effective in young people but have high death rate
in case of myelofibrosis median survival rate is 4 to 5 years.
