How Can Chronic Depression Be Better Controlled?

Sir,
I am 37 (height 165 cm, weight 71 kg). I have been sufferring from depression for quite long (first detected in 2005). I consulted many doctors and homeopaths since then, but still I feel that I haven't come out of it. The following are what I am undergoing:
(1) I don't feel like travelling long distances.
(2) I feel better when I am under Sun.
(3) I don't enjoy little moments of life as I feel others (e.g. feeling after getting promotion, increment, building a house, etc.)
(4) I don't feel too happy when I achieve something.
(5) I feel heavy headedness all the time.
(6) I don't get proper sleep and never feel fresh in the morning.
(7) I am married and have two daughters
(8) I sometimes have arguments with my wife because of my parents.
(9) My father (72 yrs) has dementia and mother has signs of depressions.
(10) Currently I am taking S-Celepra (10 mg)
(11) Is it true that antidepressant drugs have side effects (e.g. deteriorate the sexual activity, weight gain, etc)
(12) Please suggest best possible treatment as it has become really frustrating having it for quite long.

You are 37 years old and are suffering from depression. The symptoms like inability to enjoy moments of life, don't like to travel long distances, less happiness, poor and non-refreshing sleep, frequent arguments with family members etc are due to depression only. History of depression in family further increases the risk.

You are currently taking Escitalopram and this is a right drug for such symptoms. The drug is over all safe to use and is not associated with any severe side effects.

Mild side effects like nausea, dizziness, diarrhoea etc symptoms are seen. In some individuals sexual side effects can be seen at high doses like 15-30 mg per day. Delayed ejaculation, anorgasmia are common side effects. But fortunately these side effects are reversible and are not permanent. The drug is not associated with much weight gain.

If you still have symptoms with the medicines then you can also visit a psychologist for Cognitive Behaviour Therapy. The therapy will help in improving your family functioning. Adjunctive medicines like Bupropion can also be used. The drug help in augmentation of actions of Escitalopram. Consult a psychiatrist for prescription of drugs.

I went through your details. I am afraid, none of the symptoms you provided here signifies clinical depression. S-Celepra 10 mg is an antidepressant and 10 mg is the lowest possible dose. In my opinion, you are taking a placebo.

What you need is to change your attitude and thinking pattern. Learn about clinical depression and anxiety and stress. Knowledge should provide you insight about its management. Consult a psychologist for psychotherapy based therapies.
