High BP, Diabetes. Had Triple Bypass. Smoker. Scheduled CAT Scan. What Is The Cause?

My grand father is at the ER right now with a blood pressure of 212/79. The Doctor's can't seem to figure out whats causing it. So they are taking him to get a CatScan. My grandfather is a diabetic, he had triple bypass surgery Feb. 2012, and continues to take Blood Pressure pills, and continues to smoke.

What could be causing the blood pressure to rise?

Kidney is the commonest cause in such situation...
Some hormones (like epinephrine as in Pheochromocytoma) may be responsible...
He needs continued follow up by his Internist / Cardiologist...
Smoking is dangerous...Stress will contribute...
