Heart Problem, Shortness Of Breath, Hypertension, Low Platelet Count, Myoacadial Ischaemia And Cardiac Hypertrophy. Suggest

My mother is 50 years old. 154cm, 70KG, has been diabetic for 30 years. Currently not taking any medications. My mother went through ECG which interpreted as myocardial ischaemia and cardiac hypertrophy. Secondly, she got 2D-ECO Doppler & color flow imaging which resulted in “concentric hypertrophy of left ventricle” and “Good LV systolic and diastolic function”. The cardiologist said she has 160/100 BP, but when checked at home it was a little above normal. She does not feel any type of hypertension, stiff neck, or shortness of breath. But sometimes she feels palpitation. She has also been diagnosed with “collection in cul-de-sac” and “Heavy uterus”. Her blood count shows 75000 platelets and low mildly low hemoglobin concentration, interpreted as “THROMBOCYTOPENIA” Then finally she went through Exercise Treadmill Test. Test Summary: > Reason for Termination: General Fatigue > Exercise Time: 9 min 00 Sec > Max Speed: 3.4 MPH > Max Grade: 14% > Estimated Max. Workload: 10.10 METs > Peak HR achieved: 171 bpm (100% Max. Pred HR) > Max ST Depression: 3.2mm > Leads with ST depression >1mm : II, III, aVF, V5, V6 Impressions: > Exercise capacity good > Normal chronotropic and haemodynamic response. > Positive for ECG evidence of exercise inducible myocardial ischaemia PLEASE SEE THE UPLOADED REPORTS FOR DETAILS My Questions are: 1. Is there any serious risk for this type of ischaemia? The cardiologist suggested an angiogram, is that necessary and safe for a diabetic? Can this condition be cured without angioplasty or stent insertion? 2. The BP readings of the treadmill test are completely normal before exercise and after 4 min recovery, and have risen and fallen consistently during the time. The doctor hasn’t measured the BP since after the test. I researched on the internet and found out that when the diastolic value rises between 90-100, then risks are suspected. Could my mother have the white collar syndrome in this case, because when measured at home the BP is almost always normal and 140/90 at maximum. I would be very grateful to any responses. THANKS

Dear patient,

Thanks for your health concerns. To address each one of the questions:

1) Inducible ischemia carries a higher risk of cardiovascular events, including heart attack, sudden cardiac death, thus it should be investigated further to see if there are blockages that correlate with her symptoms

2) An angiogram if required, is nowadays a routine procedure with 1/1000 risk of complications and 1/10000 risk of fatality, usually safe in experienced centers. It may prove valuable, as if there are severe blockages in the arteries they could be treated with catheters

3) Medications are nowadays also very effective for treating patients with heart disease. She definitely should follow the prescription medications suggested by her cardiologist in the meantime

4) The elevation in blood pressure with exercise is a normal response; a proportion of patients do have "white coat" high blood pressure, thus the current gold standard for diagnosis is an ambulatory blood pressure monitor, that records BP during the patients usual activities.
