I fell a little over a week ago and hit my head. i lost consciousness and went to the er and my CT scan came back normal. I have been having headaches off and on since the fall. I woke up this morning with a small painful lump on the back of my neck as well as a very painful migraine . should i go to urgent care or the er? or should i just make an appointment with my GP?

It seems from the history that you had a injury to the head following which you had loss of consciousness.
This indicates concussion head injury. Since you are having headache with a lump on the head you need to get consultation by your doctor to rule out concussion head injury and treatment accordingly.

Thank you
