Have Severe Pain With Lateral Sudden Movement. Have Bursitis, Piriformis Muscle Pain And Sacroiliac Joint Pain. What To Do?

I have raging pain like a sharp stab with a knitting needle with lateral sudden movement. I have had two rounds of guided cordizone shots in a surgery center for - trochanteric bursitis, piriformis muscle pain, sacroiliac joint pain and ischiogluteal bursitis. I see a pain management doctor at UCLA and have meds that knock me out. I want to research surgery that would end the pain.
Ideas?
I am not prepared to pay money before I have an educated interaction with an MD.
