Have Chesty Cough And Throat Pain. Smoker. Breathing Problem. Suggestions?

I have a chesty cough, it hurts in my chest but when I feel the need to cough it's like a stabbing pain in the back of my throat. I need to go to sleep, but when I have a coughing fit I feel like I can't breathe. I'm a smoker (I have cut down significantly since the symptoms started) and I know that tar and gunk will settle on my chest in the night - is there a possibility I'll wake up and not be able to breathe?
