Had Mild Attack, Stents Placed, On Metoprolol, Started Retaining Fluid. Can I Take Lisinopril/ Hctz Instead?
I was put on metoprolol suc 25mg on July 5th due to very mild heart attack and having 2 stents placed. Prior to this I was on lisinopril/hctz 20/25mg 2 tabs daily. The doctors left me on the lisinopril but stopped the hctz. After 2 1/2 weeks I started retaining fluid I went from 232# to 248# over the course of that 2 1/2 weeks. is there any reasonI can take the lisinopril/hctz instead of adding Lasix and potassium which is what they want me to do.

Both HCTZ and lasix are diuretics (water pills).
Mechanism of action, potency, side effects - vary
Both may lead to potassium deficit / ACEI (lisinopril) may retain potassium.
In simple hypertension, HCTZ is better...
In heart failure, lasix is better and has a wide dose range.
The treating doctor alone can suggest better - based on his assessment of the situation.

Hydrochlorthiazide does have a few undesirable effects ,especially in cardiac patients like you.Metabolic abnormalities may occur.Hyperglycemia and hypercholesterolemia which can occur with HCTZ increase the cardiac eventualities.There is another important side effect.It can cause cardiac arrhythmia ranging from ventricular ectopy to blocks.There is a good chance that you may have started developing some ventricular ectopics,because addition of metoprolol suggests that.That may have been the reason for the change.If that is the case,then the advise given by the doctors is more beneficial for you.Lasix may have to be added to prevent fluid retention.It is commonly used drug in cardiac patients.I am not very sure about the use of supplemental pottasium.Hyperkalemia is more dangerous.So,you can take Lasix instead of HCTZ and supplement pottasium whenever its levels fall low.
