Fluid Around Uterus, Liver, Spleen And Hemorrhagic Cyst, Ruptured Ovarian Cyst. Worry About Hepatitis?

I am an ultrasonographer and had a pt. with significant fluid noted around her uterus, ovaries,(her uterus and ovaries appeared to be floating) and of course fluid extended into the culdesac. Ovaries nl size with no suggestion of collapsed or hemmoraghic cyst. Fluid also noted both flanks and liver and spleen edge. Liver has" bright" areas throughout. neg for ductal dilatation, Liver and spleen nl size. gb, panc and kidneys unremarkable. Pt is 32yr and has had 3 previous episodes and seen@ 3 different hospitals during a 2 yr period of time. During one episode pt , became unresponsive and cat scan done which mentioned fluid with the explanation given prob ruptured ovarian cyst.I have been an ultrasonographer 37 yr and today's us is not like any ovarian cyst rupture I have ever seen especially since the ovarian tissue shows no suggestion of it, Pt is in significant pain with nausea and vomiting. Any Ideas...wondered about hepatitis but that is about all I could think of

Thanks for your query.Based on the facts that you have posted it appears that your patient has what is called as haemoperitoneum secondary to ruptured ovarian cyst the ruptured site being sealed up by omentum.

However final diagnosis can be confirmed only by aspirating the fluid and sending it for cytological examination.

Please ask her to consult general surgeon for clinical assessment and further treatment.
