Hi,
Welcome to HCM,
After going through your history, there are multiple issues let me explain one by one.
First is family history of Diabetes, heart disease and lung disease(Emphysema), for which you should take care to go for medical check up so if anything is there may detected early and treated accordingly.
Your symptom of chest pain and shoulder pain cause must be investigated it is unlikely due to heart problem it is better to exclude by proper tests.
Please visit your Physician and physical examination and tests like Complete blood count, Fasting blood sugar, postprandial blood sugar,Liver functions,Kidney functions,Lipid profile,HbA1c, an X ray chest, Ultrasonography of abdomen and 2 D Echocardiography and stress test.
If anything is detected then further test like CT scan chest and for Emphysema if necessary alpha1 antitrypsin test may be done.
Pins and needle is due to compression of nerves during wrong posture if pain is severe and persist then simple x ray cervical spine and MRI can be utilised for confirmation.
Hope this helps.
Take care.
Good Luck.
