Hello, Premarin is conjugated estrogens, a female hormone for women to reduce symptoms of menopause (such as hot flashes, vaginal dryness). It rarely cause serious problems from blood clots (such as heart attacks, strokes, deep vein thrombosis, pulmonary embolism). But such symptoms have been reported by some patients. If you have symptoms like chest/jaw/left arm pain, unusual sweating, sudden or severe headache. Weakness on one side of the body, confusion, etc then you should report it urgently to your treating doctor. Long time usage of any medication is not good.
