I have reviewed your query and here is my advice.
1. Although stress test came positive but not the angiogram, get a Cardiologist's opinion regarding the same since false positive stress test can occur because of certain noncardiac issues and medicines.
PS.
Levothyroxine does cause tachycardia, palpitations, anginal pain as common ADR [adverse drug reaction], so it is advised to start with a low dose in people with pre-existing heart conditions, so get a
second opinion or cardiac Consultation to exclude any pulmonary disease or heart pathology.
2. Sometimes, an overdose of thyroxine can cause
muscle weakness, fatigue,
weight loss which can act as a contributory factor for
shortness of breath [since the body is unable to undertake physical activity], so please check with your Thyroid [TFT] values.
PS. Under no circumstances, you should stop/change/taper your dose without first consulting with your attending Physician.
