Does Indole-3-Carbinol Cause A Positive Cotinine Test?

Good evening, doctor. I test positive for cotinine in my urine for an insurance policy test and they label me as a smoker which I am not. It is a false positive. Can cotinine levels in the urine be caused from a diet high in cruciferous vegetables with the presence of indole-carbinol? Thank you

Hello,

Cotinine is the major metabolite of nicotine found in urine in higher concentrations than in blood or saliva.

Indeed, cruciferous foods, containing indol 3 carbinol if consumed prior to testing will cause false positive results.

Plus exposure to nicotine fumes can also cause false positive results. It will take 4 days for the body to get clear urine.

So try to avoid these foods, plus avoid exposure to the fumes at least 5 days prior to testing.
