Could Acid Reflux With The Feeling Of Choking Indicate Presbyesophagus?

I have recently been experiencing acid reflux while sleeping. My doctor says I have presbyesophogus but I experience the sensation of choking but am not really choking. I have to vomit up small amounts of what I have been eating to stop the sensation of choking but can go back to eating okay after this ends. Sometimes it clears immediately and other times it can take up to an hour. what do you suggest I do/

General & Family Physician 's  Response
Hi & Welcome to HCM.

I appreciate your concern for feeling of choking sensation while swallowing.

A sensation of food being stuck or slow to pass through the esophagus (food pipe) could be due to abnormal muscle movements (motility) of the food pipe resulting in acid reflux & vomiting and referred to as Presbyesophagus which needs to confirmed by tests lie barium swallow x ray, an upper endoscopy (EGD), or an esophageal motility test (manometry).

I would advise you to consult a physician or gastroenterologist for proper clinical examination and relevant lab tests as suggested above.

The effective treatment includes use of Pantoprazole (to neutralize or reduce stomach acids) and Domperidone (to improve esophagus muscle movement).

Besides medication, the following precautions are recommended.

1. Diet modification: Eat slowly, don’t talk while you eat, take small bites, sit in an upright position after meals, use a blender to puree solid foods if needed, thicken liquids with milk, juice, broth, gravy or starch to make swallowing easier.
2. Avoid smoking, alcohol, coffee and hot n spicy foods.
3. Sleep only after 2-3 hours of having dinner
4. Elevate the head end of your bed to prevent the acid reflux.
5. If you get difficulty in swallowing consult the doctor immediately.

Hope the suggestions given above could be useful in getting rid of the symptoms associated with presbyesophagus.
