Hello
I can understand your concern
Looking at your history this could be due to STI or local injury or inflammation during rough sex

if you can upload me a complete medical history and a photograph I will be able to guide you better

meanwhile you can consult your GP for examination a day a battery of tests like blood for CBC liver and renal function test urine culture

you may require a course of antibiotics under supervision of your doctor
have plenty of fluids
maintain good hygiene

thanks for your question
wishing you good health
Regards
