Constant Fever With Strong Chills, Continual Headaches, Neck Stiffness. Started Florinal. What Next ?

Thirty-nine days ago I developed a fever of 104.4 and was quite weak and tired. The fevers alternated with strong chills for a couple of days. I have also had continual headaches, tender sinuses and neck stiffness every day since then. There were no other sx, no URI, no rhinitis, no gi problems. I saw my PCP on or around Day 4 and was advised it was the flu. I took Tamilfu though it was a late start for that. Also, ibuprofen on a fairly regular schedule of three times per day. At times it was effective and at others, not. Two weeks ago my PCP gave me a Z-pak and ordered labs looking for infection/inflammation. The labwork was negative. Last week I had an ultrasound, also negative. I was started on Fiorinal 2 days ago with limited effect. What should be done next?

Thanks for the query to H.C.M. Forum.
I would come up with these possibilities for such type of fever with chills , these includes.
1 Algid malaria i.e. cerebral malaria , diagnosis can be confirmed by blood film for three consequent days for malarial parasite( falciparum ). In your case the best way is card test for falciparum strain.
2 Meningitis , diagnosis can be confirmed by C S F ( cerebro spinal fluid ) examination by lumbar puncture.
3 Encephalitis , diagnosis can be confirmed by C S F and clinical examination.
In my opinion consult a physician and get his opinion about algid malaria.
