Your smoker friend has only 1 week complaint of severe Constant
Chest Pain.
Based on the query information,2 yrs back Xray chest was normal.
If chest pain is the single complaint,I would rule out the Infective causes for such a lump after review of the Chest Xray.
I would advise CT/PET Scan to study more details of the lump in the right lung.
Till you have definite study of the lung lump with
Oncologist, who would examine all the aspect of lung lump,You should not carry any
anxiety from such a painful lump in the lower lung,unless proved by HPR of the lump biospsy Flexible
Bronchoscopy Under CT/PET Guidance or HPR study of the
Bronchial lavage from the nearby
bronchus on flexible bronchoscopy.
Plan of assessment-
-If the lump remains or increases even after Antibiotic course for 10 days time,I would not label as a Canerous lump of the lung blindly.
So till then close this fear of Cancer,that would be my advise.
