Can Banocide Forte Be Given To A Patient Showing Evidence Of Enterococcus Spp?

Patient has 79 ESR, 40% eusinophile, 9700 TLC, creatinine 2.02, urea 82 and urine culture has recently shown evidence of enterococcus spp. The patient's vitamine D3 is 10.37 currently and 9 months ago it was 23.7. Is it advisable to give this patient banocide forte? If so, what should be the dose.

Hello
You want to know the doses of BANOCID FORTE i.e. diethylcarbamazine ( D E C ).
Indicated for W. Bancrofti, W. malayi, filarial infection Tropical eosinophilia ( as you have tropical eosinophilia).
Filsriasis , toxocariasis . loiasis ( aslo for prophylaxis).
Doses as follow;
Day one : 1 mg / kg body weight in three divided doses. Thereafter increase gradually to 6 mg / kg body weight daily for 2-3 days ---for 21 days .
But this can be used for 6-12 months period .
Since you E S R is very high , serum creatinine is also elevated with blood urea and urine also contains enteococcus with low level of D3 .
All these are concerning so first of all consult a NEPHROLOGIST and get his opinion.
