Can Aten-H And Amlong Be Taken Instead Of Prolomet-AM?

My Father aged 70 was taking Aten H 50 & Amlong 5 mg from the past 7 years prescribed by our family Dr. He recently visited a Cardiologist and had ECHO and ECG test done. The cardiologist has advised him to take Prolomet AM 25 instead of the above medicine. is it safe?

General & Family Physician 's  Response
Aten H – it contains atenolol and hydrochlorothiazide and is used for treatment of hypertension.
Amlong 5 mg contains amlodipine 5 mg
Prolomet AM 25 mg contains metoprolol 25 mg and amlodipine 5 mg.
Atenolo and metoprolol belong to same class of medicine i.e. beta blockers. Both have same effect and side effect profile.
So Prolomet AM = (Aten + Amlong) minus hydrochlorothiazide
Previously you were using three medicines (atenolol + hydrochlorothiazide + amlodipine).
Now you have been prescribed only two (metoprolol + amlodipine).
You cardiologist has omitted hydrochlorothizide because it causes side effects such as low potassium, weakness, and increased uric acid level in blood on long term use. If blood pressure can be controlled by three medicines then why to use three medicines simultaneously ??
Combination of two medicines is better than three medicines in terms of safety in view of long term use.
Advice
1.     Continue Prolomet MT as suggested by your cardiologist
2.     Regular exercise at least 30 minutes for 5 days in a week. The best types of exercise for lowering BP include walking, jogging, and cycling
3.     Eating a diet that is rich in whole grains, fruits, vegetables and low-fat dairy products and skimps on saturated fat and cholesterol
4.     Consume less salt preferably less than 3 gm per day
5.     Avoid smoking, alcohol, coffee and caffeinated beverages
6.     Lose extra weight if you are overweight and keep your body mass index between 19 and 25. BMI is calculated by formula BMI = Weight in Kilograms / (height in meters x height in meters)
7.     Reduce your metal stress because stress activates sympathetic system and increases BP
8.     Measure your BP regularly and be in touch with your cardiologist
I hope this information will help you. I will be happy if you ask another question
