Based on the history you have shared, this tightening needs to be evaluated by careful examination. Some connective tissue disorders may cause such skin tightening.

Scleroderma or mixed connective tissue disorders may present like this. a rheumatologist opinion would be better in that regard. Sometime in winter due to dryness of skin , it may look like that. Some skin rashes like psoriasis etc may also present like this at start.

Meanwhile if no such lesion is present keep hands warm with gloves and use vaseline for keeping them moist and soft.

Supplements for zinc and vitamin C and E may also be helpful.
