Question: sir I have pain in my back of left testicle before 5 years..it's paining..it's create any cancer

Based on the facts that you have posted the pain at the back of your left Testicle is mostly due to infection of the epididymis (Epididymitis)

Consult qualified Urologist for clinical assessment and get following basic tests done to establish diagnosis
1)Urine Routinely and Culture
2) Ultrasound Scanning of Scrotum

In the meantime start taking antibiotics like Cefixime and Nitrofurantoin along with anti inflammatory drug like Diclofenac twice daily

It should get resolved within a week
