I can understand your concern.

The high fasting glucose despite increasing the dose (after excluding hypoglycemic events during the sleep by checking glucose level at 2.00 a.m) of basal insulin means that you have insulin resistance and an oral antidiabetic medication can be of help.

Before suggesting antidiabetic tablets, your weight, renal and liver function should be known.
If your physician has excluded the tablets due to contraindications, all you can do is to increase the dose (careful of hypoglycemia during the night) of insulin until the fasting glucose is normal.
We do not recommend to do more than 40UI of basal insulin in one injection, but for doses over 40UI, it should be divided into 2 injections (every 12 hours).

Meanwhile, the dose of rapid-acting insulin should be judged depending on postprandial glucose after fasting and pre meals are corrected.
