Question: history:I have cystic bilaterally Bronchiectasis I had a left lobectomy and lingulectomy when I was 10 years old. -Pseudomonas aeruginosa multi Antibiotic resistance.
I repeated multiple hemoptysis secondary to Bronchiectasis since 9/2018, but really large amount and been hospitalized every time with it, just discharged this week. attached an example with only one cough.

Question: this situation will be with me or this is a sign that my disease is progressing? What is the end point for this massive blood? Is it really okay! No one explain anything because I was so sick and they thought it was pulmonary embolism first from the CT.

Brief Answer:
Bronchiectasis is a chronic condition, thus, unfortunately, very likely it will be with you long term- further details on disease severity and progression below

Detailed Answer:
Hello and welcome to "Ask a Doctor" service,

I carefully read your query and understand your concern. Bronchiectasis is a condition that may greatly affect the quality of life and can be very distressing. I am sorry you are going through this, I hope my answer will give you some clarification.

Bronchiectasis is generally a chronic condition and unfortunately, very likely you will have this long term. Hemoptysis (brining up blood with cough) is a possible complication of bronchiectasis. It may be due to a possible infection (viral or bacterial) or it may be due to engorged blood vessels in the airways that burst and bleed.
Each time a patient has bleeding, it is important that the patient is hospitalized so that the infection is treated with antibiotics (if the infection was the cause) and the blood vessel is closed (if this was the cause of the bleeding).

As for the disease severity and progression, bleeding is a sign of complicated bronchiectasis but is not a good enough index to determine the progression of the disease.
There are tools that calculate the severity of the disease and may give you some insight into if the disease is progressing or not.
These are online tools for bronchiectasis severity where you plug in all the required information and they tell you how severe your condition is or you can work up one with your doctor (this is the safest option as you can have more accurate information).

I wish you good health and hope this answers your query. I remain at your disposal in case further medical assistance is needed.

Brief Answer:
Treatment is needed each time bleeding happens- unfortunately, it is not a one time fix

Detailed Answer:
Hello and welcome back,

If there is bleeding from engorged veins hospitalization is needed and endoscopic examination of the airway and clogging the bleeding vessel is needed each time it happens.
Unfortunately, there is no one time fix for all possible bleeding sites, thus, every time a patient has bleeding the procedure should be repeated.

I hope this answers your query and is helpful to you. I remain at your disposal in case further medical assistance is needed.

Regards,
Dr. Antoneta Zotaj, General and Family Physician
